This is a summary of the Beauty NP Notice of Privacy Practices. It describes how medical information about you may be used and disclosed, and how you can get access to it. You will receive the full notice at your visit, and you may request a copy at any time.
Our commitment
Beauty NP is required by law to maintain the privacy of your protected health information (PHI), to give you notice of our legal duties and privacy practices, and to notify you if a breach of your unsecured PHI occurs.
How we may use your information
- Treatment — to provide and coordinate your care, including with our collaborating physician.
- Payment — to bill and collect payment for services you receive.
- Health care operations — to run the practice, improve care, and meet our legal obligations.
- Appointment communications — confirmations, reminders, and your intake and consent forms, by email, phone, or text.
- As required or permitted by law — for example public health reporting or responding to lawful requests.
Other uses and disclosures — including most marketing uses and any sale of your information — are made only with your written authorization, which you may revoke at any time. Before-and-after photographs are never shared without your written consent.
Your rights
- Get a copy of your health record, and ask us to correct it.
- Ask us to limit what we use or share, and to contact you in a specific way or at a specific place.
- Get a list of certain disclosures we have made of your information.
- Get a paper or electronic copy of the full Notice of Privacy Practices.
- Be notified promptly if a breach affects your information.
